If you would like to go as a chaperone on any China Spring choir trip, you will need to fill out the forms that the school district needs in order to do a criminal background check. This is a district policy for all school activities. Even if you filled out these forms last year, you are required to fill out new ones this year.
Here is the process you will need to go through:
Go to the district website and look under “hot topics” on the home page (bottom right) Open the volunteer handbook (read it) and print off pages 11, 13-14, and 21. Fill them out and take them to the administration building along with your driver’s license. Or you can send me copies of all of that (front and back of license) and I will get it to the admin for you. It only takes a few days to gain approval. Once you are approved, you will be notified and will be able to chaperone ANY event in the district.
